Duties and responsibilities  Working at Redat Health Care In-Clinics and Home Care services. Traveling to patients’ homes and managing their care plans according to physicians’ instructions.Assess and chart observations of the patient’s condition at each visit.Complete evaluation tasks, including reviewing medication and vital signs.Monitoring, addressing, and documenting the symptoms and changes in patients conditions Using appropriate assessment tools regarding clients well-being and careAdminister Physician-prescribed medication.Administering medication and insulin, and completing blood pressure, glucose, urine, and stool tests.Inspecting wounds, changing dressings, and handling personal grooming and hygiene.Provide catheter careEnsure patient is fed and bathedEnsure bedding is cleanListening to the concerns of family members and answering their questions.Testing for muscle weakness, bedsores, and any signs of infection.Provide education to patients and families on proper home health care procedures and strategies.Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.Keep the patient comfortable through palliative care.Recommend tools or devices that may improve the quality of life for the patient.Performs other related task given by supervisor.
