Internal/External Vacancy Announcement Description of the VacancyCommon Vision for Development Association (CVDA) is a partner with Mekdim Ethiopia National Association (MENA) and Mary Joy in Addis Ababa Administration, Sidam region and SNNPR respectively to implement the USAID Family focused USAID Family Focused HIV Prevention, Care and Treatment Services in Addis Ababa , Sidam region and SNNPR. The main purpose is to strengthen local HIV epidemic control to achieve 95 percent of individuals living with HIV know their status, 95 percent of persons living with HIV to initiate antiretroviral therapy and 95 percent of antiretroviral therapy clients achieve viral load suppression by 2020/21. HIV mitigation services for vulnerable children will focus on case discovery and linkage to HIV treatment among undiagnosed children living with HIV, on achieving better health outcomes through HIV viral load suppression and establishing interpersonal, family and community norms to achieve HIV and violence prevention (9 to 14-year-old children).Roles and responsibilities Facilitate CHCT’s engagement at community level,Engage community members in planning, implementing and reviewing community related activities including linking with the Health Extension ProgramWork with Health facilities in linking clients to other social services in the community such as psychosocial and economic support (counseling, IGAs …)Identify cases that are going to be referred to community from health facility together with the health facility provider or case managercollaborate with health facility level case managers in identifying stable and unstable clientsEnsure linkage of clients refer from community to health facilityProvide community based case management for stable and unstable patients in consultation with the ART centers together with CRPsSupport follow up of clients at community level on appointment spacing Participate in community based service delivery model as per the MoH’s future guidance to differentiate care to clients need and preferenceFacilitate/Provide community based care and support for non-stable patients to lesson burden to health facilities and clients Assist CRPs in screening of TB, STI, Mental health and other OIs based on the need for such supportprovide technical assistance to Case managers, Social serice workers and Community resource personsTraining of Case managers and CRPs to get them acquainted with task below for Social service workersMentoring and supervising CRPs and case managers to get them acquainted with task below for CRPsReview and improve of activities of CRPs Provide technical assistance to (CCs) and (CCCs) Be part of the CCs/CCCs and provide technical assistance in their routine tasksParticipate in beneficiary identification including assessments of Households for economic vulnerability and food securityEnsure volunteers are involved in a meaningful way as per the guidance of CC/CCCs Conduct index case testing at community levelReceive list of families of index cases and apply risk assessment for testing Identify sextual networks of index cases in confidential and systematic manner Counsel high risk family members for testing and refer for health facility Conduct assisted partner notification and do community based testing based on clients’ preference. Facilitate self testOVC care and support and PreventionEnsure OVCs on comprehensive care provided service based on standard and graduate based on SOPEnsure 9-14 years OVCs on prevention package reached with curriculum based prevention training. Use and implement comCare app/mobile app for referral and linkage and case managementOversee implementation of CHCT in relevant community based activities including stigma reduction, Gender equality, women empowermentthat contribute to improve targeted testing and quality of HIV care and treatment Working conditionsThe position holder is expected to work during weekends and after hour to provide client friendly services in the community. The position requires to work with multiple actors in health facilities and communities. The task requires closely working with the case managers in ART centers and volunteers (CRPs) in the community as well as community structures such as CCs/CCCs and other social service providers. Duration of employment: form date of employment up to September 30, 2021 with high possibility of extension based on performance and availability of fund.
